Change of Personal Information
In order to keep our records up to date, we would be grateful if you could inform us of any change in your personal information, i.e., name/address/home and/or mobile telephone numbers.
We make every effort to provide the best possible service for all of our Patients. However, we are aware that sometimes things can go wrong. If you do have any cause for complaint regarding the service or treatment you receive at the Park Medical Centre or Foxhole Medical Centre, please contact the Practice Manager, Mrs D Marshall, at the Surgery address, who will try to resolve the matter as quickly and amicably as possible.
We are continually striving to improve our service and would welcome any suggestions or ideas you may have.
We offer a full range of contraceptive services including coil fitting and Nexplanon; appointments can be made by contacting reception.
If you think you may require a Doctor to visit you at home, please contact the Surgery as soon as possible.
Patients may view their medical Records in accordance with the Data Protection Act 1988. A charge of up to £50 is made for any photocopies requested. Contact the Surgery for more details.
New patients can register with the Practice by requesting a New Patient Registration Form from reception.
Non English Speaking Patients
For non-English speaking patients, access to a translation service is available.
Patient Advisory Liaison Service - PALS
This service is available to help sort out Patients problems or concerns, provide information on NHS services and offering advice and support, they can be contacted on 0161 426 5888.
If you would like to speak to a Doctor or Nurse on the telephone please ask the receptionist for a telephone consultation when you ring and they will return your call at the first opportunity. You will be placed on the appropriate telephone list and either a Doctor or Nurse will ring you to discuss your concerns.
If you require any vaccinations for foreign travel you should make an appointment with the Practice Nurse who will discuss your requirements and give you details of any charges.
Please download the attached document, complete and bring with you to your first appointment which should be at least six weeks before you intend to travel. You may then be required to make a second appointment to receive the appropriate vaccinations; this should be at least two weeks before you travel.
